What about cosmetic services and over-the-counter dispensing?  Do I need to pay you 8% on my cash business as well?

No, the contract allows you to carve out cosmetic services and product sales.  You must, however forward all medically necessary services to the billing service for processing; even if the patient has no insurance and pays cash for their clinical services.
